Adult Services

Marconi Older Adult Center

  • The Marconi Older Adult Center is located in our neighborhood in South Philadelphia at 15th and Porter Streets. They offer daily lunches, nutrition programs, cultural and social programs. The center is open to individuals over the age of 50. Transportation is also available for residents over 65. Education Classes

Diabetes Education @ Rodenbaugh Diabetes Center

  • Penn Medicine offers a 10-hour diabetes self-management education class that covers nutrition, medications, exercise, glucose monitoring, lifestyle, exercise, and coping with diabetes. Classes are offered throughout the year and are free. To register for a class call 1/800-789-7366.

Home Care Services

Custodial Services: These are services that are typically provided by a nurses aide and do not require a doctor's approval. Some duties can include bathing, dressing, light housework, meal preparation, and grocery shopping. These services may not be covered by insurance.

  • Angels on Call is a home care agency that serves the greater Philadelphia region. Offering transportation services to doctor's and personal appointments, companionship, bathing, cleaning, laundry, grocery shopping, menu planning and meal assistance. For more information Click Here

In Home Support Program/Elder Care Program

  • St. Monica's Parish located directly across the street from our office offers a program to assist elderly parishioners in their homes as they age. This program offers support both short and long term with a host of services including case management, in-home safety assessment, to light housekeeping or errands. This program also helps to prevent the elderly from feeling the social isolation that can accompany the aging process. For more information contact Bonney DosSantos at 215-334-0285. Please note this program is only available for St. Monica's parishioners.

Skilled Nursing Care

Skilled Nursing Services: These services are considered medically necessary and are provided by a licensed medical professional. Skilled nursing services require a doctor's approval. Some duties would include wound care, physical therapy, occupational therapy, catheter care, and IV medications.

  • This independently owned and operated organization has served South Philadelphia for over 20 years. Continuous offers a patient-centric communication to our office and families so continuity of care can be achieved. CHC is JCAHO certified and offers skilled nursing, home health aides, physical therapy, occupational therapy, speech therapy, and has social work on staff. For more information Click Here

  • Angels on Call is a home care agency that serves the greater Philadelphia region. Angels on Call offers skilled nursing, wound care, physical therapy, occupational therapy, and speech therapy. This organization is also a VetAssist program participant to get many needed services to our veterans and their spouses. Angels on Call also participates in the waiver programs. This organization also offers custodial services to provide continuity of services in home.

Medication Support/Pharmacy Support

  • Centennial Pharmacy is an independent and locally owned pharmacy that offers medication packaging called CarePack's which offers daily and time of day sorting for patients with multiple medications to assist in sorting. CarePacks are an excellent resource for patients to reduce the opportunity for missing a dose of medicine. They also offer free home delivery throughout the Philadelphia area and South Jersey, convenient for patients who have a difficult time getting to the pharmacy. Centennial Pharmacy also offers vaccines, immunizations and educational classes for patients.

  • Packer Apothecary, located in the Packer neighborhood of South Philadelphia, is a locally owned pharmacy that provides door-to-door delivery, prescription savings and even convenient blister packaging to avoid the pill bottles and have everything organized for each patient. Known to the community for excellent and friendly customer service, Packer Apothecary is a great and local option for patients.

  • PACE is a government sponsored program that offers prescription cost assistance to qualifying patients.

  • AZ&Me is a prescription savings program sponsored by AstraZeneca that supports patients who cannot afford the cost of their brand medications.

  • Janssen Care Path is a prescription drug savings program sponsored by Janssen to offer support to patients who are unable to afford the cost of their brand medications.

Transportation Needs

  • CCT Connect Ride Share program for senior citizens and residents of Philadelphia to ride door to door for a reduced cost. Reservations must be made in advance and riders must have exact change. For more information Click here

  • To Enroll for CCT Connect Click here

  • This innovative company uses Lyft and Uber to request rides for older adults without a smart phone by calling 855-464-6872 to immediately request a car from your home. Go-Go Grandparent allows family members to receive text alerts to keep track of the loved one's location making it easier to stay in the loop. You can also schedule rides for future appointments all through the phone. A nice feature for older adults when calling is requesting a car for the last drop off location to skip remembering addresses. For more information please check out their website.

  • Logisticare is the Philadelphia county Medical Assistance Transportation Programs's service provider for patients who are enrolled in Medicaid and Medicare and medically disabled. Reservations must be made 3 days in advance. All reservations are made through the Department of Human Services by calling: 1-877-835-7412.

Insurance Assistance & Social Assistance

  • This organization works to assist caregivers in many different aspects of the care spectrum. CARIE offers support for legal assistance, elder abuse support, housing security, problem resolution, transportation problems, Nursing Home complaints and resolution, and Medicare beneficiary fraud. Philadelphia Corporation for Aging (PCA)

  • PCA is a local organization in Philadelphia that helps to arrange services for aging residents. PCA has a number of programs spanning from food and meal support, home care, and housing resources to legal assistance and transportation needs.

  • The Philadelphia Department of Public Health offers many services to those who cannot afford insurance as a safety net for care. These services include both acute and chronic care. The DoPH also provides options for vaccinations, city health centers, and assistance with addiction. For more information visit their wensite.
